How do you answer a customer's supplier quality questionnaire?

Summary

Answering a supplier quality questionnaire takes 4 steps: find evidence per item, attach its type and date, mark unconfirmed items, then assign owners and deadlines. The method holds for any customer form, portal or self-assessment, as long as each answer is tied to a controlled document you can hand over at the customer's next audit.

  • ISO 9001:2026, the sixth edition, was published on 16 September 2026 and replaces ISO 9001:2015 (ISO, 2026).

  • Organizations certified to ISO 9001:2015 have until 30 September 2029 to complete the transition, and from 31 March 2028 new accredited certifications may only be issued to ISO 9001:2026 (ISO/TC 176/SC 2, 2026).

  • ISO does not issue certificates; a certificate from an accredited certification body can be verified in the IAF CertSearch database or with the certification body (ISO, 2026).

  • ISO 9001:2026 keeps the Harmonized Structure, so its clause structure has not changed from ISO 9001:2015 (ISO, 2026).

This method covers quality questionnaires a customer sends to a supplier of chemicals, lubricants or process fluids, whether first approval or requalification. Regulatory questionnaires (REACH, RoHS, PFAS, conflict minerals) and sustainability ratings need their own evidence rules and are not covered here.

What do you need before you answer a supplier quality questionnaire?

Before you answer a supplier quality questionnaire, you need four things fixed: what the customer is asking about, which of your sites and products the answer covers, the deadline, and who in your company owns each section. A questionnaire answered before these are clear tends to mix group-level facts with site-level questions.

  • The request itself. A sustainability adviser's guidance on supplier questionnaires says to "save a copy of the request" and keep "the email, portal screenshots, deadline, customer name, legal entity, requested reporting period, help text, and any scoring guidance" (Keslio, 2026). The same applies to a quality form.

  • The answer boundary: the legal entity, the manufacturing or blending site, and the product families the customer buys.

  • The customer's evidence rules: which documents they want attached, and any age limit for test reports or audit results. If the form states no rule, ask before you start.

  • An owner list: quality for certification and nonconformance items, production for change control and traceability, the laboratory for test methods and calibration, sales or customer service for contacts and delivery.

  • Your evidence register: the current certificates, procedures, audit reports and product documents, each with its revision and date.

Which evidence answers each item of a supplier quality questionnaire?

Each item of a supplier quality questionnaire is answered by one controlled, dated document, and the questionnaire section tells you which kind. A buyer's training guide for its own questionnaire describes the quality system section as covering "certifications, regulatory compliance, and 3rd party audit questions", along with calibration control, materials and supplier control (Creation Technologies, 2021).

The map below decides which document to pull for the sections that appear on most chemical supplier questionnaires.

Questionnaire section

Evidence that answers it

What to check on it

Certifications

Certificate from the certification body

Standard and edition, scope, sites, expiry

Change control

Change notification procedure and a recent notice sent to customers

Revision date, notice period stated

Traceability

Lot numbering procedure and one sample trace from raw material to shipment

Lot on label, certificate of analysis and delivery note match

Nonconformance handling

Nonconformance and corrective action procedure, one closed record

Root cause, action and closure date recorded

Calibration

Calibration procedure and the register for test equipment

Due dates not passed

Product documents

Safety data sheet, technical data sheet, specification

Revision matches what the customer holds

The certifications row needs the most care. A guide on checking certificates warns that "the organisation has only been assessed (and, hence, certified) against part of the activities it undertakes" in some cases (Designing Buildings), so read the scope wording and the site list before you tick Yes.

For product documents, cite the revision the customer will receive with the next delivery, not the copy saved with an earlier questionnaire. For procedures, one closed record next to the procedure shows the procedure is used, which a procedure alone cannot show.

How do you show the evidence type and date in a questionnaire answer?

You show the evidence type and date in a questionnaire answer by writing the answer, then the document that proves it, with its number, revision and date, then the scope it covers. One line per item is enough, and the reviewer at the customer can find each document without asking.

A workable answer line has five parts, shown here with illustrative content:

  1. Answer: Yes, No, Partial, Not applicable or Not yet confirmed.

  2. Evidence type: certificate, procedure, record, test report or data sheet.

  3. Identifier and date: document number, revision and issue date, for example "QP-07 rev. 4, 2026-03-12" (illustrative).

  4. Scope: the site and product families the evidence covers.

  5. Attachment or reference: the file name sent, or "available at audit".

Edition and expiry belong in the line for any certificate. A certificate to ISO 9001:2015 stays usable during the transition, but organizations certified to that edition have until 30 September 2029 to move to ISO 9001:2026 (ISO/TC 176/SC 2, 2026). Write the edition printed on the certificate, not the edition the form names, and add the expiry date so the customer can see when the answer goes out of date.

Where the form gives only a Yes or No box, put the evidence line in the comment field. A sustainability adviser warns that "A questionnaire answer without evidence can look weak even when the answer is true" (Keslio, 2026).

What do you write when a questionnaire item cannot be confirmed yet?

When a questionnaire item cannot be confirmed yet, you write "Not yet confirmed" and state what will be checked, who will check it, and by which date, instead of guessing Yes. A guessed Yes becomes a written commitment, and the customer's auditor can test it against your records at the next audit.

Use four answer states, and keep them apart:

  • No: you do not have the procedure, record or certificate.

  • Not applicable: the item does not apply to your products or site, with one sentence saying why.

  • Partial: the item is met for some sites, products or periods; name which.

  • Not yet confirmed: the evidence may exist but nobody has checked it.

Keslio's guidance draws the same line for sustainability forms: "Use 'yes' only when the answer is true for the requested scope and period" and "Use 'not applicable' only when the topic genuinely does not apply to your business or the assessed entity" (Keslio, 2026).

A Not yet confirmed line reads, for example: "Not yet confirmed. Laboratory to confirm whether the viscosity method is run to the current edition; owner: QC lead; answer by 2026-11-06" (illustrative). Send the open items to the customer as a list with the questionnaire, then send the confirmed answers by their dates. A deadline you set and keep reads better than a Yes you later retract.

How do you check a supplier quality questionnaire before you return it?

You check a supplier quality questionnaire before you return it by testing every answer against six checks: evidence named, evidence date, certificate scope, certificate status, answer state, and owner for open items. Run the checks with someone who did not write the answers.

The table below decides whether each questionnaire answer is ready to send.

Check

Pass

Fail and what to do

Evidence named

Document type, number, revision and date given

Bare Yes: add the evidence line or change to Not yet confirmed

Evidence date

Within the age limit the customer set

Too old: replace with a current record or state the date and ask if it is accepted

Certificate scope

Scope and site list cover the product and site supplied

Site or product missing: answer Partial and name what is covered

Certificate status

Valid in IAF CertSearch or confirmed by the certification body (ISO, 2026)

Not found or expired: correct the answer before sending

Answer state

One of Yes, No, Partial, Not applicable, Not yet confirmed

Blank or "TBC": choose a state and add a reason

Open items

Each has what to check, owner and date

Missing owner or date: assign them before sending

The certificate scope row catches the most common error: a group certificate quoted for a site that is not on its site list. Answer Partial and name the covered sites.

Sign-off follows the owner list: each section owner confirms their answers, and one person accountable for quality, usually the quality manager, signs the whole return. In Lubinpla, a reply to a customer's technical inquiry answers each item from your own records and documents, and marks an item it cannot support as unconfirmed instead of guessing, in one cited document you review before it is sent.

How do you reuse past questionnaire answers without copying outdated evidence?

You reuse past questionnaire answers without copying outdated evidence by keeping an answer library in which every answer carries its evidence date and a recheck date, and by rechecking the evidence each time an answer is reused. The answer text can travel between customers; the evidence behind it has to be current on the day you send it.

Build the library one row per recurring question, with five fields: the question in your own wording, the answer, the evidence document and revision, the evidence date, and the date the answer must be rechecked. Set the recheck date from the document itself: a certificate's expiry, a procedure's review date, or the customer's age limit for test reports.

Before each reuse, run three checks:

  1. Wording. Keslio advises that you "can reuse the same evidence base, but you should still check each customer's wording" (Keslio, 2026). A question about one site is not answered by a group answer.

  2. Revision. Compare each cited document with your document control register, and update the line if a newer revision exists.

  3. Edition. ISO 9001:2026 was published on 16 September 2026 (ISO, 2026), so lines that quote an ISO 9001 edition need review as your certificate moves to the new edition.

Mark a reused answer with the date it was last checked, so the next person knows how old it is.

Frequently asked questions

Who should sign off a supplier quality questionnaire?

Each section owner signs the answers in their area, such as quality, production or the laboratory, and one person accountable for the quality system signs the full return. That person should be able to produce every cited document at an audit. Sales can coordinate the return but should not sign quality answers alone.

Can you send your standard self-assessment instead of the customer's form?

You can offer it, but ask first. Some customers accept a standard self-assessment with a cover note that maps it to their sections; others score only their own form or portal. If they require their form, reuse the answers and evidence from your self-assessment, not the document itself.

Should you attach every document the questionnaire mentions?

Attach what the form asks for and list the rest as "available at audit" with its number and date. Procedures with confidential formulation or process detail can be shown on site instead of sent. Say this in the answer, so the item is not scored as missing.

What if a customer asks about a clause of ISO 9001:2015?

Answer it. ISO 9001:2026 keeps the Harmonized Structure, so the clause structure has not changed (ISO, 2026). State the edition on your certificate, and check the sub-clause number against the 2026 text before you quote it after your transition.
